- If name differs from primary identification, please present one of the following (must be an original or certified copy): marriage certificate, court order, or divorce decree (must be accompanied by proof of prior name if not listed in decree)
- When changing to a new name, you must obtain an updated social security card and wait at least one business day before applying for a license or ID card Â
- If you are a first-time applicant who has had name changes (marriages/divorces) you must present documentation showing a complete chain of names since birth (unless you are presenting a US Passport listing the desired name)

Note: If your last Florida driver license renewal was completed online, your next renewal must be done in person and requires two proofs of Florida address, even if your license is REAL ID–compliant.
Canadian Citizens presenting only their valid passport are entitled to a driver license or ID card issuance for six months. Canadians with USCIS/DHS documents are entitled to issuance until the specific expiration date of their document(s), not to exceed one year.

United States lawful permanent residents (LPRs), also known as ‘green card’ holders, are non-citizens who are lawfully authorized to live permanently within the United States.

- Valid foreign passport and proof from United States Citizenship and Immigration Services (USCIS), such as approved I-797 receipt, immigration judge’s order, or stamp in passport showing that asylum or refugee status is granted/approved

Note: Those applying for a license will be issued a temporary driving permit (en español) (an kreyòl). The permit should be carried with photo identification until the hard card comes in the mail. Please wait until the last week the permit is valid to return to our office. That should allow enough time for the license to be approved and mailed from the Florida Department of highway Safety and Motor Vehicles.
